Importing Medical Devices Is Far More Than Just Delivering Goods to the Port
Many clients come to us with the first sentence: "Manager Zhu,we want to import a diagnostic device from Germany; the goods are ready, just help us with customs clearance and transportation." Every time I hear this opening,I will first ask the client to sit down, make a cup of tea,and then tell them a fact: For medical devices, especially Class II and III devices, the real challenge starts from the moment you place the order overseas.Customs clearance and transportation are just the final "physical work"; more than 80% of the workload and risks ahead are concentrated on "access qualification" and "compliance preparation".This is not general cargo; it is a special commodity directly or indirectly used on the human body. The regulatory logic of China Customs and the drug administration is completely different from importing a batch of screwdrivers.

Over the past few years,I have seen too many cases: the goods arrived at Shanghai Port, but the Medical Device Registration Certificate was not yet issued, leading to high storage fees in the bonded warehouse day by day; or because the Chinese manual and labels did not meet NMPA’s format requirements, they were ordered to be returned for rectification, resulting in losses of hundreds of thousands of yuan back and forth. Our role is to eliminate all these risks before the goods are shipped.
Core First Step: Figure Out Which NMPA Class Your Product Belongs To
This is the cornerstone of all work.China implements classification management for medical devices, and the category directly determines the registration path, cycle, and cost. The CE or FDA classification provided by many overseas manufacturers cannot be directly applied to China; it must be determined based on China’s Medical Device Classification Catalogue.
| Management Category | Risk Level | Common Product Examples | Registration/Filing Form | Estimated Cycle (2026 Benchmark) |
|---|---|---|---|---|
| Class I | Low Risk | Surgical Scalpels,Medical Cotton Swabs,Examination Gloves | File with Municipal Drug Administration | 1-3 months |
| Class II | Medium Risk | Blood Pressure Monitors,Electrocardiographs,Intermediate Frequency Therapy Devices,Some Diagnostic Reagents | Apply for Registration with Provincial Drug Administration | 6-18 months (depending on product and testing conditions) |
| Class III | High Risk | Cardiac Pacemakers,Artificial Joints,Vascular Stents,Implantable Devices | Apply for Registration with National Medical Products Administration (NMPA) | 18-36 months or longer |
You must save this table.Last year,we assisted a Mr.Qu in importing a set of laser treatment equipment. The overseas manufacturer insisted it was a "Class I device",but based on its working principle and intended use,we initially judged it to be at least Class II in China.Later,we insisted that the client entrust a qualified domestic testing institute to conduct a pre-classification determination, and the result confirmed it was indeed Class II.If we had prepared according to Class I,the entire project would have faced the catastrophic consequence of starting over.
Intertwined Registration and Import Processes: Understand Key Nodes at a Glance
The import of medical devices is a deep interweaving of the drug administration’s registration approval process and the customs clearance process.Ideally, they should proceed in parallel and seamlessly connect.A typical agent import process for medical devices involving registration has the following core nodes:
- Pre-consultation and Product Definition: This is the most easily overlooked "soft cost".You need to clarify the product’s Chinese classification, registration path, whether clinical trials are required, and whether the overseas factory has a quality management system that meets Chinese regulatory requirements (e.g.compliance with Chinese GMP).
- Appoint Domestic Agent: Overseas applicants must appoint a Chinese domestic enterprise legal person as an agent to handle product registration and assume joint liability. This role is crucial; many subsequent communications, document submissions, and emergency handling depend on them.
- Registration Testing and Clinical Evaluation: Products need to be tested at domestic testing institutions recognized by NMPA.Class II and III products usually require clinical evaluation materials, which may mean conducting clinical trials in China—this is the main source of time cost.
- Preparation and Submission of Registration Materials: Compile a large number of technical documents, including product technical requirements, risk management reports, clinical evaluation materials, etc.and submit them to the corresponding drug regulatory authority.
- Registration Review and Certificate Issuance: The drug regulatory authority conducts technical review, and upon approval, issues the Medical Device Registration Certificate. At this point, the product obtains the qualification to be sold in the Chinese market.
- Port Compliance Preparation Before Import: After obtaining the registration certificate and before shipping the goods, you must complete: 1) Verify that the information on the registration certificate (model specification, production address) is completely consistent with the actual goods; 2) Prepare complete and compliant Chinese manuals, labels, and packaging marks; 3) For active devices, confirm that the power supply standard meets Chinese standards; 4) Apply for the Import Medical Device Customs Clearance Form (commonly known as the "Commodity Inspection Form").
- Customs Clearance Upon Arrival: Declare to customs with documents such as the registration certificate, clearance form, contract, invoice, and packing list.Customs will focus on reviewing the price, classification, and verifying whether the physical goods match the documents.

You see,the actual sea or air transportation of goods is just a physical link interspersed in the above paperwork. Many agent companies only handle the last segment, but we move our services forward to the "product definition" stage to control the overall situation.
Three Major Risks to Watch Out for in 2026 and Practical Pitfall-Avoidance Suggestions
Based on current project experience,I summarize three risk points that are most likely to cause problems and result in huge losses:
Risk 1: Mismatch Between Registration Certificate and Physical Goods
This is the most common problem in port inspections. For example, the approved model on the registration certificate is "Device Pro 2025",but the actual imported goods have "Device Pro 2025 Plus" printed on the outer packaging or nameplate.Adding one word, customs and the drug administration can it as an "unregistered model"—at best, requiring an application for model change and the goods being detained at the port; at worst, direct return or destruction. We handled an endoscope host where the overseas factory upgraded a non-critical software before delivery and added "Ver 2.0" to the nameplate without permission, almost leading to the entire batch being detained.Later,we urgently coordinated with the overseas factory to re-make the nameplates and arranged engineers to replace them in the bonded area, which resolved the crisis. Therefore,it is essential to clearly require the overseas factory in the contract to provide samples of physical labels and manuals for the domestic agent to check before shipment.
Risk 2: Non-Compliant Chinese Labels and Manuals
NMPA has extremely detailed format and content requirements for Chinese manuals and labels of medical devices. Many importers directly use translated versions of overseas manuals, which is far from sufficient. They must be reorganized in accordance with the Regulations on the Management of Medical Device Manuals and Labels.Especially content such as "contraindications","precautions",and "warning information" must be prominent and unambiguous. We once encountered a case where the warning statement in the client’s self-translated manual was not strong enough, and the drug regulatory officer required rectification, resulting in all printed manuals and packaging boxes being scrapped.
Risk 3: Inadequate Implementation of UDI (Unique Device Identification) System
China is fully promoting the UDI system for medical devices.By 2026,most Class II and III medical devices will need to have UDI marked on their smallest sales unit and higher-level packaging. This UDI code needs to be filed in the database of China’s drug administration and linked to product registration information. Many overseas factories are not familiar with this; either they do not print the code,or the printed code does not meet China’s GS1 or Zhongguancun standards. The goods cannot pass commodity inspection after arriving at the port. Our approach is to guide overseas factories to apply for UDI that meets Chinese requirements during the registration stage and pre-review the code printing samples in advance.
Why Choose Shanghai Port and the Value of Professional Agents
Shanghai Port has unique advantages for medical device imports: customs and drug regulatory officials have rich experience, stronger handling capabilities for new technologies and complex products, and relatively high communication efficiency. We are located in Shanghai and can obtain the latest port dynamics and regulatory interpretations in the first place. But this does not mean we only operate in Shanghai.Relying on our national port linkage network, whether your goods enter China through Tianjin,Shenzhen,or Chengdu,we can provide services of uniform standards through remote declaration collaboration.
The real value of a professional agent lies in risk control and resource integration. We are not just document transmitters. When there is doubt about product classification,we can mobilize resources from cooperative testing institutes for pre-judgment; when the registration review encounters a supplementary notice,we can assist you in communicating efficiently with registration consultants to understand the core concerns of the drug regulatory officers; when the goods encounter inspection doubts at the port,we can appear as professional agents with a full set of reference materials to explain. These are the keys to preventing your project from getting stuck.
Finally,a honest word: Agent import of medical device registration is a long-distance race that requires patience, professionalism, and careful planning.Expecting to find a cheap agent who only handles customs declaration will likely result in hidden costs and risk costs far exceeding your imagination.Building a compliant and reliable framework from the beginning is the fastest and most cost-effective path.
